HL7 Cross-Paradigm Information Sharing for Electronic Long-Term Services & Supports (eLTSS), Release 1 - US Realm
DESCRIPTION
The purpose of the eLTSS informative document is to inform stakeholders interested in enabling standards-based electronic sharing of Long-Term Services and Supports (LTSS) service plans, and more specifically, to guide those developing and implementing software solutions in the LTSS ecosystem.
The scope of this informative document is to describe how Home and Community-Based Services (HCBS) data requirements, documented in the 56 data elements included in the eLTSS Dataset, can be represented for exchange and sharing using nationally recognized content and vocabulary standards.
The eLTSS Dataset was crosswalked to the Fast Healthcare Interoperability Resources (FHIR) and Consolidated Clinical Document Architecture (C-CDA) standards, as well as to any FHIR or C-CDA mandated or recommended code sets for specific data elements. This informative document summarizes the findings from this crosswalk regarding how well FHIR and C-CDA align with the eLTSS Dataset, and identifies any gaps discovered during the mapping process. Detailed mapping information is provided in the appendices.

DEVELOPMENT BACKGROUND
The eLTSS Initiative is a joint CMS and ONC project initiated in 2014 as one of the four components of the Medicaid Testing Experience and Functional Tools (TEFT) demonstration grant program. It is facilitated through a public collaborative platform, the ONC Tech Lab—previously referred to as the Standards & Interoperability Framework—and includes participants from the six TEFT-grantee states (Colorado, Connecticut, Georgia, Kentucky, Maryland and Minnesota), and the broader HCBS community to include service providers, advocates, consumer representatives and HCBS system vendors.
