Please note that you will need certain resources in order to complete this form. In particular, you will need the following:
Project Scope Statement: You will need to reference the PSS in order to complete several sections of the
form. A link to the PSS is available in the Work Group Information section of the form, and will open in a new window.
Referencing the PSS will allow you to:
Verify that the Ballot Name and identified project family are appropriate based on project information;
Verify the Realm (either Universal or Realm-specific;)
Verify whether or not the material was externally developed;
Identify other cosponsors of the Project;
Correctly provide a general description of the material for use in the Ballot Announcement and intended for
a general audience; and,
Complete the ANSI-Required Information section of the form.
WG Approval of the ballot: On the form, you will need to confirm that your WG has voted to bring this material forward to ballot.
A Listing of Major Changes from Reconciliation (if appropriate): While you do not need to list every change since the material last balloted,
you will need to provide a high-level, general listing of major changes. Simply providing a statement such as "Changes from Reconciliation"
or "See Reconciliation Spreadsheet" is NOT sufficient, and such forms will be rejected as incomplete.
Other WG Information: In addition, you will need other information either from the members of your WG or your
publishing facilitator or editors, including:
The level at which the material should be balloted;
Whether or not your WG will be requesting alternate ballot open and close dates (as these would need approval from the TSC);
The name of your publishing facilitator;
The name of your primary WG contact for any ballot-related questions;and,
The email addresses of any additional people who should receive duplicate Vote Confirmations (these go to the
WG cochairs by default).

effective in the 2016 January cycle we have added the Naming Conventions for your reference.
Ballot Readiness Status: to improve the conceptual/visual towards ballot approval for Project and Ballot item reviews; the status of the progress of requirements includes the steps for project approval, prior ballot reconciliation, draft and final content deadlines, and review of final material prior to ballot open.
