Description | This project will create a domain analysis model (DAM) for capturing and managing pressure ulcer prevention information, including pressure ulcer risk assessment and prevention interventions. The primary use case is the pressure ulcer risk assessment and the planning of preventative interventions, with the primary expectation for implementation (not in scope) being the patient transfer in an IHE e-Nursing Summary. Several additional activities may occur, depending on the availability of resources. The domain analysis model may contain certain packages that will be specified to a semantic level; i.e., they will contain concept code bindings. These packages will be proposed as 'candidate detailed clinical models' to the Patient Care group. As the primary use case is an US Nationwide Health Information Network (NHIN)-supported interaction carried by an existing format, the project may also conduct preliminary analysis necessary to communicating the e-Nursing summary in a Clinical Document Architecture document. |

Project Need | Pressure ulcers are one of the most common 'never events' identified by the National Quality Forum. Efforts to prevent them and address them consistently across settings will significantly improve quality of care while providing significant cost savings. There is a lack of a standard method and structure to capture and communicate information regarding pressure ulcers. Thus, the effective driver for the project is to capture and communicate reliable information, including information about pressure ulcers, about the state of a patient being transferred from one facility to another. The structuring of the information necessary to support interoperability will also support other activities requiring predictable information - system configuration, clinical decision support, and quality analysis. This work has been begun at the US Veteran's Health Administration in collaboration with Clinical LOINC and Kaiser Permanente; the VHA would like to see this information standardized in order to support interoperability among providers. CDA implementations could support care coordination, transitions of care, patient transfers, internal longitudinal assessment, and quality assessment; implementations in other formats may support clinical decision support or other system interactions. |
