Description | With cooperation from CDC and National Healthcare Safety Network (NHSN) partners, this project will develop implementation guide(s) constraining HL7 Version 2.5.1 for purposes of reporting microbiology results, ADT, and pharmacy data. The implementation guides will support transmission of electronic surveillance data to the National Healthcare Safety Network. This is intended to be a US Realm only project. BACKGROUND From the inception of the NNIS system in 1970, CDC has enabled collection and analysis of bacterial pathogen and antibiotic resistance data by individual healthcare facilities participating in CDC's surveillance system. The data entered by individual facilities into the system are used by CDC for aggregate, national-level analysis. However, the burden of manual data entry for bacterial pathogen and antibiotic resistance data has limited the extent to which these data are available for analysis by participating facilities and by CDC. The increasing availability of these data in electronic form, coupled with advances in technical capacity for electronic data exchanges between healthcare facilities and CDC, provides the impetus for a standards-based, electronic messaging solution that will enable facilities to report antimicrobial use and resistance data without having to resort to manual data entry. Health Level Seven (HL7) is the prevailing healthcare information technology standard for electronic messaging of clinical and laboratory data. The eSurveillance message specifications have been designed to overcome the shortcomings of manual data entry. The electronic transfer of eSurveillance data will allow CDC to: - Increase the reliability of data. - Reduce the burden of data collection. - Expand the areas within a facility where data is collected to provide a clearer understanding of antimicrobial resistance and disease transmission. - Provide an opportunity to conduct national surveillance of antimicrobial use and resistance in many facilities to identify national trends and emerging resistance. The eSurveillance 2.5.1 messages provide specification for the transfer of microbiology, pharmacy, as well as Admission, Discharge and Transfer (ADT) data. The microbiology message contains all antimicrobial susceptibility tests on organisms identified in cultures as well as genetic tests such as MecA for MRSA. The pharmacy message contains all orders, dispenses, returns and administrations depending availability of data in the pharmacy information system. The ADT data contains basic demographic information needed for analysis as well as providing the location in the facility where a patient resides during sample collection and antibiotic administration. The existing HL7 CDA R2 project for Healthcare Associated Infection Reports (HAI II) focuses on the electronic representations of NHSN event forms and the associated event data. Some of this HAI data may be available electronically as 2.x messages in a healthcare enterprise. The eSurveillance HL7 2.5.1 messaging project is focused on capturing existing electronic data flowing in a healthcare enterprise and transmitting that data to CDC using 2.x messaging infrastructure already in place in the healthcare enterprise. Algorithms may be applied to this data to determine NHSN events and replace much of the need for manually entered data that is required for existing NHSN surveillance methods. |
