Description | In 2010, the Public Health Informatics Institute (PHII), through a grant funded by HRSA, convened a workgroup to develop a Newborn Screening (NBS) Dried Blood Spot (DBS) implementation guide for creating an HL7 Version 2.5.1 ORU^R01 message for reporting a standardized NBS DBS laboratory result, based on guidance, vocabulary, and an example HL7 version 2.5.1 message developed by the Health Resources and Services Administration (HRSA), the National Library of Medicine (NLM), the Regenstrief Institute, and the International Health Terminology Standards Development Organisation (IHTSDO). The PHII implementation guide was written in 2011 and has been implemented at various levels by different state newborn screening programs in the United States, but was never officially submitted to HL7 for balloting, and was not updated. During the interim, NLM has worked with state newborn screening programs, HRSA, APHL and ACHDNC to update the Newborn Screening LOINC panel (which includes SNOMED CT codes for conditions and other 'answers') and the example HL7 version 2.5.1 results message, available at: https://newbornscreeningcodes.nlm.nih.gov/HL7. The purpose of this project is to include implementer feedback and update to using the LRI Framework as basis for exchanging a standardized NBS DBS laboratory result and then ballot as a profile of the LRI Framework (currently R1D2). This message will send all electronic NBS DBS laboratory test results from the laboratory that conducted the testing to the results receiver. The results receiver may include one or more of the following: the primary care physician, birthing hospital, public health agency, medical specialist, health information exchange, or vital records department. This implementation guide will also provide recommendations on vocabulary standards and on types of identifiers to capture, which HL7 fields those identifiers are mapped to, and the specific identifiers that may be used. |

Project Need | The Newborn Screening Technical Assistance and Evaluation Program (NewSTEPs) is a HRSA funded program of the Association of Public Health Laboratories (APHL), in partnership with the Colorado School of Public Health (CSPH), is the national resource center and data repository for state and territorial newborn screening programs in the United States. NewSTEPs was tasked by HRSA to update and formally ballot the HL7 NBS DBS implementation guides for reporting laboratory test results and submitting lab orders that were written in 2011 by PHII. The current version of the implementation guide for creating an HL7 Version 2.5.1 ORU^R01 message for reporting a standardized NBS DBS laboratory result has been implemented at various levels by different state NBS programs, but was never officially submitted to HL7 for balloting. The inconsistent and variant use of the current version of the implementation guide has emphasized the current need of an official HL7 certified specification for sending electronic laboratory NBS DBS test results, and recent emphasis on certain topics in NBS, such as the timely reporting of results, has increased the demand for HIT interoperability solutions to improving NBS processes. |
