HL7 FHIR® Implementation Guide: Post-Acute Care Cognitive Status, Release 1- US Realm
DESCRIPTION
The PACIO Cognitive Status Implementation Guide (IG) is for exchanging cognitive status and mental functioning observations primarily involving post-acute care (PAC) transitions between various healthcare settings, including ambulatory care, acute care, long-term post-acute care (LTPAC), and home- and community-based services (HCBS).

BENEFITS
The benefits of this project include:
- Producing the data model required to comprehensively exchange cognitive status information between health care settings,
- Developing FHIR Implementation Guides, Profiles, and necessary Extensions to specify how to syntactically and semantically exchange that data between care settings, and
- Improving patient and provider communications and supports access to longitudinal health information that enables improved efficiencies, improved quality of care, and improved health outcomes, allowing data to be usable across the continuum of care, and beyond the traditional healthcare system – into the community.
IMPLEMENTATIONS/CASE STUDIES
- The work of the Cognitive Status IG was led by the Post-Acute Care Interoperability (PACIO) Project Team engaging with community to develop this standard mechanism for EHR interoperability to improve patient care management. Many stakeholders (both clinical and technical) support PACIO, including MITRE, Centers for Medicare and Medicaid Services (CMS), Office of the National Coordinator (ONC), and over 30 representatives from the health care industry.
- The implementers included MITRE, Altarum, Michigan Health Information Network (MiHIN), Patient Centric Solutions, Inc., and MatrixCare.
DEVELOPMENT BACKGROUND
The Cognitive Status IG has gone through successful testing in five separate HL7 Connectathon events in 2020 and 2021.
Open-source Reference Implementation server applications and client applications were developed and made available for interested parties to participate in interoperability testing.
