DESCRIPTION

The purpose of a Questionnaire Form Definition document is to capture the health survey questions or question sets to be administered to a patient. Questionnaire Form Definition documents enable the definition of questions for surveying the patient’s perceptions on their health and the impact that any treatments or adjustments to lifestyle have had on their quality of life. The Questionnaire Form Definition documents may carry a variety of clinical and non-clinical questions and branching logic in order to present the patient with a dynamic health survey for assessing health status including, but not limited to, the patient’s functional, cognitive, and physiological characteristics.
