HL7 Version 2.5.1 Implementation Guide: Laboratory Orders (LOI) from EHR, Release 1, STU Release 4 - US Realm
DESCRIPTION
The Laboratory Orders Interface Initiative focuses on identifying the requirements, specifications and standards, and on providing the implementation guidance for electronic ordering of laboratory tests in the US Realm. The scope of the Laboratory Orders Interface Use Case includes requirements to enable a particular implementation of Electronic Health Record System (EHR-S) to use standardized structured data in a defined inter-organizational laboratory transaction. The Use Case requirements are directed at laboratory test orders between an Ambulatory Provider’s EHR-S and a Laboratory’s Laboratory Information System (LIS). Future versions of this guide may harmonize with existing guides to extend interoperability of laboratory results across care settings, e.g., acute care.
The “HL7 Version 2.5.1 Implementation Guide: S&I Framework Laboratory Orders from EHR, Release 1 DSTU Release 2 - US Realm”, referred to as the Laboratory Orders Interface Implementation Guide (LOI IG) is a companion guide to the “Version 2.5.1 Implementation Guide; Orders and Observations; Interoperable Laboratory Result Reporting to EHR (US Realm), Release 1 STU Release 4” referred to as the Laboratory Result Interface IG (LRI IG) and the “HL7 Version 2 Implementation Guide: Laboratory Test Compendium Framework, Release 2 DSTU 4” referred to as the electronic Directory of Service IG (eDOS IG). These provide guidance on how to apply Version 2.5.1 of the standard, and select pre-adopted capabilities through Version 2.9, to the exchange of laboratory orders from the EHR to the receiving laboratory in an ambulatory setting. This implementation guide focuses on key points of broad interoperability, including use of strong identifiers for key information objects and use of vocabulary standards. This version supports additional data elements needed for newborn dried bloodspot screening (NDBS), Public Health reporting (PH) including pandemic response requirements, the ability to request withholding results reporting to patients/caregivers until the provider had the opportunity to share those results, and references to preliminary guidance to include SOGI/Gender Harmony data..

DEVELOPMENT BACKGROUND
The LOI implementation guide was developed by the Lab Work Group through an initiative supported by the Office of National Coordinator (ONC) Standards & Interoperability Framework, and project sponsored by HL7’s Orders & Observation Work Group.
Subsequent DSTU updates were published in December 2013, September 2015, April 2018 and these are replaced by this version published October 2022.
